✅ Certificate of Coverage is non-transferable.

Dengue RX Plus for Adults

✅ Available for individuals 21 to 65 years old.
✅ Valid for 1 year from activation date.
✅ Coverage starts 30 days after activation.
✅ Certificate of Coverage becomes non-transferable upon activation.

📝 Clients may cancel this health plan within 15 days from receipt of the Certificate of Coverage, provided the plan has not been used or availed.

💵 Refunds are subject to PhilCare's Termination Policy.

For DigiMed Consultation

• 📱 Download the HeyPhil DigiMed App from the Google Play Store or Apple App Store.
• 🔑 Log in using the username and temporary password sent to your registered email. Change your password for security.
• 🩺 Go to DigiConsult.
• 📝 Enter your Chief Complaint or reason for consultation.
• 📎 Upload your latest prescription or laboratory test results if available. Otherwise, proceed after agreeing to the Terms and Conditions.
• ✅ Click Request Consultation to join the queue.
• 📞 Wait for your DigiMed doctor's call. Calls may come from an unregistered phone number while your submitted information is being reviewed.
• 💻 Attend your teleconsultation with the DigiMed doctor.
• 📧 Check your registered email for your medical assessment and prescription.
• ⭐ Rate your consultation experience through the HeyPhil app.

For Dengue Hospitalization

• 🏥 Present your Certificate of Coverage (COC) together with one (1) valid government-issued ID at any PhilCare-accredited hospital.
• 📋 The hospital will coordinate with PhilCare's Contact Center to verify your membership eligibility and coverage.
• 🩺 Coverage approval is based on the final medical diagnosis. An initial Dengue diagnosis may qualify for an In-Patient Letter of Authorization (LOA), but the LOA may be revoked if the final diagnosis confirms that the illness is not Dengue.
• 🔄 If the final diagnosis is not Dengue, your unused Dengue benefit may be reactivated by a PhilCare Customer Care Representative.

📄 A soft copy of your Certificate of Coverage and complete policy contract will be sent to your registered email. You may also request a copy by emailing order@philcare.com.ph.

💵 If you do not agree with the Terms and Conditions and the product has not yet been registered, you may request a refund subject to PhilCare's Return, Refund, Replacement, and Cancellation Policy.

✅ You certify that all personal and health information you provide is true and accurate. Any material misrepresentation may result in cancellation of membership, denial of benefits, or recovery of payments advanced by PhilCare.

🔍 You authorize PhilCare to verify and investigate the information you have provided whenever necessary.

🕒 Coverage begins only after successful registration and the applicable activation period. Any illness, injury, or condition occurring before the coverage start date is not covered.

• ⏳ Dengue RX Plus for Adults – Coverage starts 30 calendar days after successful registration and activation.

🏥 Dengue benefits are subject to PhilCare's evaluation based on the registration date, activation date, final medical diagnosis, hospital billing, professional fees, and applicable policy exclusions.

🔒 PhilCare will collect, process, and protect your medical information solely for legitimate healthcare and business purposes in accordance with applicable privacy laws.

📖 Coverage is also subject to PhilCare's General Exclusions and Limitations and the complete terms stated in your Certificate of Coverage.
